Wholesale Account Application

Please enclose with your application a letter of intent on official ministry or business letterhead. In addition, please submit one or more of the following with your completed application:

• a copy of the applying entity’s 501(c)(3) tax-exempt certificate
  
• a copy of the applying entity’s incorporation certificate
  
• a copy of an official notice of assignment of a federal employer identification number (FEIN) to the applying entity from the U.S. Internal Revenue Service
  
• if the applying entity is a retail bookstore, a copy of the business license
